Humanitarian Heart programs are community-based initiatives dedicated to enhancing the health and well-being of families, as well as the economic stability of communities. These initiatives identify the best practice solutions for interventions aimed at reducing the incidence of chronic disease and its associated risks among underserved populations. Through evidence-based research findings, Humanitarian Heart provides system-thinking implementation strategies to efficiently increase the target population’s access to innovative and culturally appropriate interventions related to nutrition, weight management, and stress management, thereby improving positive health outcomes. Evaluations of the initiatives revealed that the biopsychosocial approach to promoting community health interventions enhances the effectiveness with which program participants respond to tailored outreach methods. The use of the biopsychosocial approach enhances community involvement by recognizing the unique barriers to lifestyle change experienced by the population being served.
